Evaluation Process

Child Find

Child Find requires all school districts to identify, locate, and evaluate all children with disabilities, regardless of the severity of their disabilities, pursuant to Education Code Section 56300.
 
Students are eligible for special education when assessment and evaluation results demonstrate that the degree of the pupil's impairment requires special education, adversely affects educational performance, and cannot be corrected without special education and related services. If your student is found eligible, an individualized education program (IEP) will be offered as part of a free appropriate public education (FAPE) upon enrollment at a South San Francisco Unified School District (SSFUSD) public school.

How to Request an Assessment

For Preschool Age Students (ages 3-5):
 
If you have a preschool age student whom you would like to begin the evaluation process to determine your student’s eligibility for special education and/or related services at a SSFUSD public school, please complete the Preschool Intake Packet online.
 
The following information and documents are required to complete this referral. Incomplete packets will be returned.
 
Information:
  • Home Language Survey
  • Developmental History Report
  • Home Health Inventory
  • New Student Registration Form
  • Emergency Card
Documents:
  • Verification of Residence:
  • Current utility bill in parent’s name or Approved Residency Affidavit
  • Birth Certificate
  • Physician’s Report
  • Updated Immunization Records
  • Any reports from previous assessments

For Students Enrolled in SSFUSD Schools:
 
Any parent suspecting that a child has exceptional needs may request an assessment for eligibility for special education services through the school principal.
 
For Students Attending Private Schools:
 
If you have a student enrolled in a private school whom you would like to to begin the evaluation process to determine your student’s eligibility for special education and/or related services at your SSFUSD public school of residence, please complete the Private/Parochial Special Education Initial Intake Packet online.
 
The following information and documents are required to complete this referral. Incomplete packets will be returned.

Requests for Evaluation During Summer Breaks

Please note that when South San Francisco Unified schools are on summer break that special education assessments and IEP meetings will be on hold. All services and assessments will resume when school starts.
